A Duchesne County nurse charged with negligent homicide made her initial appearance in 8th District Court on Wednesday. 50-year-old Jana Clyde was charged in September by the State of Utah Attorney General with Negligent Homicide, a class A Misdemeanor, in the death of Madison Jensen in the Duchesne County Jail on December 1st, 2016. In a press release from the Duchesne County Sheriff’s Office this fall, it was stated that before her death, Madison Jensen’s family had reported Madison to law enforcement authorities and she was taken into custody and then died at the Duchesne County Jail as a result of complications related to chronic heroin use. “The assistant Utah attorney general who is prosecuting the case against Ms. Clyde has informed Duchesne County Sheriff David Boren that his agency’s independent investigation determined this was an isolated incident involving one inmate and one sheriff’s office employee,” shared the Sheriff’s release. In court on Wednesday, Clyde’s attorney stated that Clyde would like to proceed with a preliminary hearing. That hearing was scheduled for
