On Wednesday, March 22nd, Good4Utah news broadcast an investigative report on the deaths of two women 4 months ago in the Duchesne County Jail with the focus on the grieving parents question, “Why did our daughters die in the Duchesne County Jail?” On Thursday, Duchesne County Sheriff Dave Boren issued a statement in response. “In November 2016, Tanna Jo Fillmore was found unresponsive in her jail cell from what appeared to be a suicide by hanging,” shares Sheriff Boren. “…I was immediately made aware of this tragic event and requested an investigation into the incident by an outside agency.” Tanna’s husband Dustin Beveridge was contacted and informed of the tragic news. Vernal City Police Department took on the investigation and Tanna’s body was sent to the State Medical Examiner’s Office where it was determined the death was in fact a suicide. The investigation by Vernal City Police also concluded that the incident was a self-inflicted hanging. Sheriff Boren also states that a GRAMA request was submitted to the Sheriff’s Office on March 9th by the Salt Lake Tribune for incident reports on Tanna Jo Fillmore, which were provided on March 16th, and no other requests have been received since. Sheriff Boren’s statement also addresses the death of Madison Jenson, who was discovered unresponsive in her jail cell on December 1st. “The cause of death was not immediately apparent and, as of today, has still not yet been determined,” shares Sheriff Boren. “As with Tanna Fillmore, a request was immediately made to the Basin Critical Incident Commander for an outside agency to conduct an independent investigation. Again, this was done to avoid any bias or other form of impropriety whether actual or perceived.” Uintah County Sheriff’s Office took on the investigation and the investigation is still in process. The State Medical Examiner’s Office has also not completed their reports that would determine cause of death, making it premature, says Sheriff Boren, to reach any factual conclusions until all the reports are complete, reviewed and considered. Sheriff Boren offered his condolences to both families.
